Student/Fellow Member Group Registration

Register two in-training members and get the third in-training member registration free. To Qualify: Must register by May 22:
• Hotel accommodations MUST be booked through ENDO 2013 Housing Bureau or offer will not be valid (does not apply to local registrants)
• Everyone in the group must be from the same organization, same department and same city
• Group registration payment must be made with one transaction
• Maximum number of free registrants is four (4)
• All forms must be submitted at the same time

Payment

A completed Registration Form, along with full payment, your credit card number (American Express, VISA, and MasterCard accepted), or check must accompany your registration in order to be processed. Registrations without full payment will not be processed. If you are faxing your registration, do not mail or you may be double charged.

Cancellations and Refunds

Cancellations must be made in writing by May 22, 2013. Cancellations made by this date will receive a full refund, minus a $50 processing fee. Requests for changing Premium Package will not be honored after May 22, 2013. No refunds will be issued for cancellations received after May 22, 2013 or for no-shows. All refunds will be processed after ENDO. Send requests for refunds to:
