Group Registration

Payment
A completed Registration Form, along with full payment, your credit card number (American Express, VISA, and MasterCard accepted), or check must accompany your registration in order to be processed. Registrations without full payment will not be processed. If you are faxing your registration, do not mail or you may be double charged.

Letter of Invitation
If you require a Letter of Invitation to participate in ENDO 2012, check the appropriate box on the Registration Form and one will be forwarded to you with your confirmation letter.  Pre-registration is required.

Cancellations and Refunds

Cancellations must be made in writing by May 30, 2012. Cancellations made by this date will receive a full refund, minus a $50 processing fee.  Cancellations made May 31-June 22 will receive a 50% refund of registration.  No refunds will be issued after June 22.  Requests for changing Premium Package will not be honored after May 30, 2012.  No refunds will be issued for cancellations received after this date or for no-shows.  All refunds will be processed after ENDO, minus a $50 USD administrative fee.
