Those registered after April 5, 2011 will not be mailed their badge material. If you register before April 5, 2011 but do not receive your badge in the mail, you may pick it up on site at registration. Please note, badges are mailed to US attendees and select international countries. All other attendees must pick up their badges on site at registration.

The ENDO 2011 Session Library features more than 100 hours of clinical and translational content from ENDO, available online and on DVD-ROM. The Session Library is available only with Premium Package Registration now through June 3. Select the Premium Package, and get the best rate for the ENDO 2011 Session Library ($125 for members and $175 for non-members).

Onsite prices for the Session Library increase to $200 for Members, $275 for Non-Members, and $150 for In-Training Members. 

STUDENT/FELLOW MEMBER GROUP REGISTRATION INSTRUCTIONS

REGISTER TWO IN-TRAINING MEMBERS AND GET THE THIRD REGISTRATION FREE

To Qualify:  Register by MAY 11, 2011.  For those requiring housing, hotel accommodations MUST be booked through ENDO 2011 Housing Bureau or the offer will not be valid. Everyone in the group must be from the same organization, same department, and same city.  Group registration payment must be made with one transaction.  Maximum number of free registrants is four (4).  All forms must be submitted at the same time.  All of the students in a group registration need to be members of The Endocrine Society. EXHIBITOR REGISTRATION

Exhibitor registration must be made online (available December 3).  

PAYMENT

Full payment (check or American Express, VISA, and MasterCard are accepted) must accompany your Registration Form in order to be processed. If you are faxing your registration, do not mail or you may be double charged.

LETTER OF INVITATION

If you require a Letter of Invitation to participate in ENDO 2011, check the appropriate box on the Registration Form and one will be forwarded to you with your confirmation letter.  Pre-registration is required.

In order to obtain AIFA authorization, Italian Pharmaceutical Companies wishing to Sponsor ENDO should refer (as imposed by Italian Ministry of Health) to the official Italian Agency for ENDO 2011:

CANCELLATIONS & REFUNDS

Cancellations must be made in writing by MAY 11, 2011. Cancellations made by this date will receive a full refund, minus a $50 processing fee. Cancellations made May 12-June 3 will receive a 50% refund of registration. No refunds will be issued after June 3, 2011. Request for changing Premium Package will not be honored after MAY 11, 2011. No refunds will be issued for cancellations received after this date or for no-shows. All refunds will be processed after ENDO, minus a $50 USD administrative fee.
