Please note the evaluation section is mandatory if you wish to receive a CME Certificate or Certificate of Attendance. Once the evaluation is completed, you will be able to print your CME Certificate and/or Certificate of Attendance. You will be required to enter your conference ID number in order to access your personal record. This has been sent to you via email.
