Note The objectives of the HSSAP initiative are to develop and exchange knowledge that will inform health services and systems across the spectrum of care for older Canadians. This care spectrum extends from primary care to long term care, home care and end-of life care. The expected outcomes are changes in policy and programming by provincial governments, health care delivery organizations and health care providers, etc. that are informed by the funded research and the knowledge translation and exchange.
